The social worker based at the hospital will offer direct assistance to those impacted by violence.

The social worker will evaluate client requirements, create and put into action therapy strategies, and offer continuous assessment of provided services.

The person who gets this job will work closely with the Senior Vice President for Community-Based Services. This individual must demonstrate a firm dedication to serving the public.

Duties & Responsibilities

Offer hands-on healthcare support to patients through evaluations, therapy sessions, and emergency assistance.
Create, put into practice, and assess personalized therapy strategies according to each client's requirements.
Take part in interdisciplinary group sessions and case conferences.
Input direct and indirect services into Simple Practice and document them in the Tower Health EPIC system when necessary, all within 2 to 3 business days following the provision of the service.
Keep up with comprehensive and prompt grant documentation as per TLC and Tower Health regulatory requirements.
Engage in local community engagement and programs aimed at preventing and intervening in acts of violence.
Gather client data to start building case files and developing case plans, along with performing intake evaluations.
Arrange services for clients, encompassing therapy sessions, healthcare provision, legal support, and various essential resources.
Support clients and serve as a bridge between them and community service providers and organizations.
Encourage and facilitate dialogue among Tower Health, TLC, and local resources.
Offer crisis intervention and defuse situations when necessary.
Recognize clients and their family members who require extra assistance with trauma and offer them preventive measures, referrals, and therapeutic interventions.
Offer home-based services to client families when required.
Follow the ethical and legal guidelines of social work practice.
Engaged in continuous professional growth through education and training programs.
Carry out additional tasks as assigned by the SVP of Community-Based Services.

Skills & Knowledge

Experience delivering clinical services in violence prevention and trauma-sensitive care
Experience in managing cases and coordinating patient care
Deep knowledge of the psychological and social impacts of violence and trauma
Strong abilities in both verbal and written communication.
Capability to collaborate with varied teams and handle difficult circumstances

Education and Experience

A graduate degree in social work.

At least 2 years of experience in dealing with violence intervention or providing trauma-informed care.

It's a full-time role requiring 40 hours of work each week, with shifts from 11 AM to 7 PM, Monday through Friday.

The school-based counselor/social worker offers initial therapeutic interventions and counseling services at a technical school.

They offer case management along with mental health assistance both within the school and inside the houses of students who are considered at risk.

The social worker/counselor will offer consulting services to school staff, act as a bridge between the school and external organizations, and conduct training sessions for both educational and community groups.

Services to students include:

Recognize students who may require trauma assistance and offer them preventive measures, screenings, referrals, and therapeutic interventions as needed.
Offer trauma-sensitive therapy and counseling services (including group, individual, and/or family sessions) to students who are at risk.
Offer mentorship along with various support programs for minors and their households who've faced traumatic events or could potentially encounter them. This includes individuals from lower economic backgrounds, people without stable housing, those within the foster care network, persons engaged with law enforcement, jobless folks, anyone coping with psychological health issues or addiction disorders, as well as students likely to leave school prematurely or currently lacking enrollment in one.
Offer case management services for students and families who need various resources.
Offer high-quality trauma assistance and mental health treatment.
Offer in-home support to households with students who have faced or could face trauma, particularly those dealing with economic hardship, homelessness, involvement in the child welfare system, or engagement with the juvenile justice system.

Support for educational staff encompasses:

Help educators implement tactics for managing student conduct.
Give employees crucial details to enhance their understanding of elements like cultural background, societal influences, economic conditions, family dynamics, and health issues that can impact a student's performance and conduct.
Educate school personnel about trauma-sensitive strategies for teaching to create secure, supportive, and caring educational settings that reduce and address the impacts of trauma.

Services to districts include:

Offer guidance on school law and policies, covering aspects of IDEA and Section 504.
Help in crafting effective positive behavior intervention approaches.
Help create and put into practice educational initiatives, covering topics such as safety measures and anti-bullying strategies.
Create substitute initiatives for students who dropout, skip school frequently, engage in misconduct, among others.

Services to parent/families

Collaborate with parents to help them effectively support their children's adaptation to school life.
Reduce familial tension so the child can perform better at school and within their community.
Help parents get access to and make use of educational tools and support systems within schools and communities.
Address the issues within a child’s home environment that impact their behavior and performance at school, as well as their interactions in the broader community.

School Community Liaison:

Gather and manage community resources to address students' requirements.
Deliver healthcare services and implement intervention methods by managing the assistance offered through qualified applicants along with coordinated care groups, governmental health agencies, non-profit youth service bodies, and locally based organizations.
Encourage and support dialogue among the educational institution, local residents, and police authorities.
Enhance accessibility to high-quality trauma-aware assistance programs and mental healthcare by connecting the community with nearby trauma-support networks and behavioral health systems.
Assist school districts in obtaining sufficient assistance from social and mental health services.
Promote the development and enhancement of community and school services aimed at addressing the requirements of students and their families.
Engage family members, educators, and community groups to support the child’s learning within their educational setting as effectively as possible.

Qualifications for this role encompass education and experience as follows:

Full-Time Licensed Behavior Specialist

This role focuses on facilitating behavioral changes. Its primary duty is to offer hands-on support to educators, school administrators, professionals from various agencies, and parents in tackling behavior modification and managing difficult student behaviors.

The Behavioral Consultant offers one-on-one and group training sessions, performs and assists with functional behavioral evaluations and positive behavioral intervention strategies, guides administrators and employees on optimal procedures and guidelines, collaborates closely with faculty and staff from schools and organizations, and delivers specialized knowledge on modifying behaviors for challenging situations.

The Behavior Consultant offers services and documentation that adhere to federal, state, and local guidelines. Additionally, these services and documents are grounded in established best practices, empirically supported techniques, and resources endorsed by the PDE.

Skills and Knowledge

Offer guidance to personnel via observations, functional behavioral assessments, creation of behavior intervention plans, consultative sessions, and suggestions tailored to the specific requirements of individual pupils. Use evidence-based approaches, validated research methods, along with endorsed procedures from the Pennsylvania Department of Education.
Carry out functional behavior evaluations for pupils within the school environment, at their homes, or in community settings.
Develop personalized positive behavior support strategies to tackle the specific behaviors.
Educate personnel, families, and pupils about the execution of the strategy. Keep track of adherence to the treatment protocol and offer input as necessary.
Create and manage continuous gathering of behavioral data, and provide consolidated reports as required.
